I create quite a few striped bg's in Photoshop, but with a simple task like this, it's just so quick to make subtle changes. I think most of these resources can be very helpful, especially when creating wireframes and mockups for clients. You can quickly generate something just for the illustration purposes. I rarely use graphic services like these, not because they're bad but mainly because I forget about them. That plus I prefer doing most things myself. Although I did come across a good form maker once that I used for a site http://www.phpform.org - worth checking out. And also if I was doing a questionnaire I would use http://wufoo.com or http://www.formspring.com rather than wasting time making my own form. Nice examples Jon.
